Responsible for providing quality patient care and performing technical skilled care. Advises patients on health maintenance and disease prevention. Serves as an advocate and liaison for and on behalf of client's rights and benefits. Coordinates services to form a support system which allows a lifestyle which is least restrictive and most desirable to the client.
- Provide professional nursing services under the direction of a Registered Nurse (RN).
- Handles all incoming calls regarding patients under Case Manager's care.
- Coordinates client services regarding patients under Case Manager's care.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Confer with nursing staff to assess patient needs, plan and modify care and implement interventions. Participate in care coordination activities and discharge planning as appropriate.
- Maintain accurate, detailed reports and records.
- Measure and record patients' medical information and vital signs.
- Monitor all aspect of patient care, including diet and physical activity.
- Supervise medications taken in home by patient and provide training to caregiver for medication setup as required.
- Communicate any problems with the patient, family or physician to the Supervisor.
- Prepare patients for and assist with examinations or treatments.
- Prepare equipment or supplies and ensure that stock of supplies is maintained.
- Instruct individuals and families on topics such as health education, disease prevention, and health improvement programs.
- Arrange crisis and respite care.
- Attends scheduled IDG meetings and develops written documentation of IDG Meeting to develop Plan of Care.
- Is available to all disciplines involved in patient care during business hours in case of problems or questions concerning patients.
- Participate in in-service meetings.
- Work on-call schedule as assigned.
- Other duties as assigned.

Requirements
- Graduate of an accredited Licensed Practical Nursing Program.
- Current LPN license from the Arkansas State Board of Nursing.
- Home health care experience preferred.
- Valid driver's license.
- Current CPR certificate.
- Ability to travel out-of-the area and overnight.
- Personal smartphone is required.
Salary Description
$22.00/hour
