Location: Chota Community Health Services
Position: LPC or LCSW

Position Summary: The LPC or LCSW provides diagnosis and treatment of mental and substance use disorders of individuals, couples, families, and groups in an outpatient setting (primary care clinic and intensive outpatient program).
Role and Responsibilities:
    • As a condition of employment, the healthcare provider will undertake to provide covered services, including emergency treatment, to a health center patient within the approved scope of project of Chota Community Health Services (CCHS), and to any individual who is not a patient of CCHS when asked, called upon, or undertakes, at or near any CCHS location and as a result of a non-CCHS patient's emergency situation, to temporarily treat or assist in treating that non-health center patient.
    • Collaborate with healthcare professionals to plan or provide treatment. Participate in Patient Centered Medical Home (PCMH) Care Team collaboration
    • Provides clinical consultation to primary care providers regarding behavioral health diagnoses
    • Provides diagnosis and treatment of mental or emotional disorders of individuals, couples, families, and groups utilizing recognized modalities
    • Provide assessment, diagnosis, and treatment of students and families in a school setting
    • Codes visits appropriately using the appropriate CPT and DSM5 codes
    • Consults other Behavioral Health staff, as appropriate, for patients with difficult or challenging situations
    • Acts as a resource for CCHS staff regarding mental health or SUD questions and assist with educating staff, clients, and the community regarding SUDs and mental health.
    • Provide emotional and mental health counseling to groups or individuals
    • Build a strong rapport with patients and make accurate assessments of their problems
    • Keep detailed, concise records of each client during the course of their treatment
    • Assist with substance use disorders (SUD) and mental health intensive outpatient program (IOP) development as well as the development of policies/procedures related to SUDs and mental health treatment.
    • Function as liaison between outside substance abuse treatment centers and mental health centers. Expedite client referral to outside centers and enable patients that come from outside centers to establish outpatient care with CCHS
    • Work with local judicial and legal services to establish a drug/recovery court that enables legal pressure to expedite clients recovery from substance abuse
    • Provide the most up to date and evidence based treatment for CCHS patients with mental health substance use disorders.
    • Functions under approved counseling protocols. Adheres to policies and procedures.
    • Develops individualized substance use disorder and mental health treatment plans.
    • Monitor client's condition and progress, reevaluate treatments, and update treatment plans on a regular basis.
    • Refer patients to other practitioners, other community resources, or higher levels of substance abuse treatment when necessary.
    • Obtains and maintains credentialing by insurance companies. Is familiar with the various insurance plans Center wants to be providers with, follows their agreements regarding referrals, pre-certification, prescriptions, hospitalization, and other utilization requirements and expectations.
    • Must be comfortable with maintaining patient safety, including, but not limited to, conflict resolution, de-escalation, calling the authorities when necessary and other appropriate crisis management.
    • Conducts self in a professional manner, maintaining adherence to ethical values at all times.
    • Regular and predictable attendance
    • Ability to cooperate and work well with others
    • Attend monthly staff meetings
    • Performs other duties as assigned or requested
Qualifications and Education Requirements:
    • Graduate from an accredited college or university with a Master's Degree as a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW)
    • Valid Tennessee license/certificate to practice as a Level II Licensed Alcohol and Drug Abuse Counselor preferred
    • Must have knowledge and experience with, but not limited to: A&D, SUD, Psychiatric, Behavioral Health and/or Mental Health patients. Chota Community Health Services
    • Maintain continuing education (CE) required by licensing/certifying board.
    • Have a valid driver's license
    • Maintain CPR certification
