Looking for Vent Certified LVNs for Private Duty

Continuity Care Home Nurses, Inc. is a Private Duty Home Care agency specializing in catastrophic care, worker's compensation clients, g-tubes, trachs, psych patients and patients with TBI. We have been providing quality home care to our patient population for over 30 years. We provide services in Los Angeles, Orange, Ventura, San Bernardino and Riverside Counties.

We are getting busier than ever and need to hire additional compassionate and committed healthcare professionals to provide quality care for our patients.

You will need to meet the following minimum qualifications:

• Current CA license in good standing.
• Minimum of one year of experience (acute setting preferred) with your credential.
• Current American Heart Association or American Red Cross CPR card
• Current Physical and TB test results
• Must be willing to provide services in patient home setting

You will be responsible for:

• Providing one to one care for the patient in their home setting
• Patient Assessment and coordination of care according to the Plan of Care, including medication administration
• Thorough documentation of care provided
• Timely submission of documentation to the agency
