Responsibilities:
Under the supervision of the Long-Term Care (LTC) Manager, the Long-Term Care Supervisor is responsible for the daily oversight of the LTC Care Management unit providing direction, organization, and monitoring of all care management services.
- Responsible for direct supervisory oversight of staff conducting care management screening, care coordination, care management and other supportive functions.
- Provides unit and departmental leadership.
- Serves as a subject matter expert for LTSS training for internal care teams and external audiences.
- Directly oversee the care coordination function within the department.
- Maintains operational processes, policies and procedures to support LTSS care delivery.
- Ensure quality and performance metrics are consistently met regarding LTSS, as specified in the contract.
- Performs and remains knowledgeable in all workflow and timeliness requirements for all areas in LTSS.
- Assists with the daily operations of licensed and non-licensed professionals.
- Monitors staff work assignments and makes appropriate adjustments based on staffing levels, staff experience, and number/request types.
- Assist the LTSS Manager in providing oversight for operational reporting compliance.
- Provides coaching and counseling to improve productivity of staff members.
- Works collaboratively and in alignment with other health and social services across the continuum of care striving to enhance the quality of Member care and services.
- Reviews quality audits and shares audit results in a timely manner with associates, providing necessary education and counseling to improve performance.
- Works collaboratively with the Manager and identified leadership to develop and implement performance measures, and monitors associates placed on performance improvement plans.
- Stays current with ACFL policies and procedures, Medicaid and LTSS requirements.
Education/Experience:
- Bachelor's Degree in a human service, social science, or health field required. Minimum of 5 years of experience in case management, at least 1 year of which shall be related to the elderly and disabled population
- Master's Degree in a human service, social science, or health field preferred. Minimum of 2 years of experience in case management, at least 1 year of which shall be related to the elderly and disabled populations
- Minimum of 1 year of supervisory or leadership experience preferred.
- Previous leadership experience including training and performance management.
- Ability to provide informed advocacy.
- Ability to interact with health care professionals in a professional manner.
- Knowledge of and experience working with vulnerable populations.
- Knowledgeable in preparation of educational materials, presentations, and overall adult learning methodologies.
- Knowledgeable in basic data analytics as related to aggregating, distinguishing, and displaying data for reporting.
- Proficiency with Microsoft Office Suite (Word, Excel, Power Point).
