Title: Long-Term Care Coordinator

Location: 33404, 33407, 33418, 33415 and any zip with a city of West Palm Beach address

Duration: 6 months contract

Target Start Date (based on successful onboarding completion): 04/15/2024

Shift: 8:00 AM - 5:00 PM - Monday - Friday

Day-to-Day Responsibilities of this Position and Description of Project:

• Managing a caseload for healthcare members with long-Term Care needs.
• Monthly and quarterly member contact will include 80% travel. Remote role. Will require a driver's license.
• Member assessments and notes. Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development
• Monitor delivery of services and follow-up with members, caregivers, or providers through in-person visits and telephonic contact
• Authorize and coordinate referral for services
• Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care
• Assist in coordinating the development of informal or voluntary services to integrate into the member care plan Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-Term Care services
• Assist members with filing and resolving complaints and appeals

Required Skills/Experience/Education:

• 2+ years of care management experience and Bachelor’s degree OR 4+ years of care management experience and Licensed Practical Nurse
• Microsoft Office and electronic medical health records experience required
• Licenses/Certifications: Valid driver's license required
• Bilingual Spanish / English is Highly Preferred
• Home health, discharge planning, or long-Term Care experience preferred

Job Type: Contract

Salary: $25.00 per hour
