LMSW, Social Worker - Case Management
This job is located in Grand Haven, MI
Job Status:
Full time, 80 hours every two weeks, 1st/2nd shift, variable
Part Time, 8 to 16 hours per week, 1st shift,
Casual, as needed
This position is responsible for assessment, treatment and Case Management to NOCHS patients. The position assists in the collaboration and implementation of a plan of care for the patient. This position provides care coordination and discharge planning.
What are the job requirements?
Current Michigan MSW license. Full license required.
Minimum of one year experience in Medical Social Work including associated hospital experience preferred but not required
Comprehensive knowledge of community resources, current reimbursement information, and federal/state laws
What are the job responsibilities?
Coordinates a discharge plan for patients.
Ability to perform conflict resolution where and when appropriate.
Identifies and participates in the development of a discharge plan to reduce nonacute days.
Monitors and records delays in care and discharge; intervening as appropriate.
Documents social work intervention and discharge planning.
Collaborates with a multidisciplinary discharge planning team for early identification of high-risk patients, initiating patient/family conferences to develop the post-hospital/discharge care plan.
Fosters role as advocate via ongoing assessment of patient needs to promote successful discharge plan.
Functions as reference and liaison for community resources and services available to patients of NOCHS.
Understands the requirements of mandated reporting standards of all suspected abuse and neglect regarding NOCHS patients.
