SUMMARY
The Licensed Practical Nurse OTP works under the supervision of the Executive Vice President of Medical Services and is responsible for dispensing any medications the clinic may utilize in the treatment of patients along with ensuring the completeness of all medical aspects of medical records to assure appropriate, competent, and efficient medical care for patients. This is an hourly position with hours and schedule established to complete the required duties. This position is not exempt from wage and hour regulations as defined by the U.S. Department of Labor and is therefore granted protections under the Wage and Hour Division of the Fair Labor Standards Act.
EDUCATION, EXPERIENCE, LICENSE, CERTIFICATION
Required:
• Active Ohio LPN license in good standing
• Two years of nursing experience
• Schedule flexibility, including the ability to work weekends
Preferred:
• Associate degree in nursing or a medical related field
• Experience in a psychiatric or substance use treatment setting
• CPR certification
• CPI certification
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Administration of methadone medication to clients, assure proper recording of dosage as well as any changes.
2. Monitor medication inventory and reconcile inventory of Methadone and Suboxone at conclusion of dosing.
3. Sign and process physician orders. Enter notes and input flags per provider order.
4. Assure patient compliance with agency policies relating to methadone medication. Perform duties in accordance with applicable professional ethics and standards and established agency policies.
5. Administer PPD’s.
6. Process phase change requests.
7. Record all doses dispensed and account for all medications used during shift.
8. Accurately maintain and record patient details in the medical records system (EMR).
9. Participate in treatment team and regular medical meeting as per licensure standards.
10. Monitor tuberculosis, HIV and Hep C testing for all patients.
11. Be proficient with state licensure standards, CARF standards, 42 CFR, and HIPAA standards and agency policies and comply with all state and federal laws.
12. In collaboration with the team, ensure a comprehensive health history is obtained and appropriate referrals are made, as needed, for further assessment and treatment.
13. Provide education to patients, family members of patients and other staff members regarding medication, symptom management, wellness and health.
14. Provide ongoing assessment of patient’s mental illness or substance use and physical symptoms as well as their response to treatment; communicate these changes to the treatment team and make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided.
15. Review health history questionnaires and follow up with clients as appropriate.
16. Provide direct clinical services to clients to monitor symptoms, carry out prescriber orders and teach behavioral symptom-management techniques to alleviate and manage symptoms.
17. Witness urine screens and process appropriately.

• Ability to learn and utilize the agency’s electronic tools effectively and efficiently within 30 days of hire.

• Demonstrate the ability to customarily and regularly exercise discretion and good professional judgment
PHYSICAL REQUIREMENTS

• Must be able to lift to 15 pounds at a time.
• Ability to perform job responsibilities in facilities or community-based residences and buildings with multiple levels and without elevator or mechanical support.
• Prolonged periods of sitting at a desk and working on a computer.
• Effectively operate a computer and keyboard.
Compensation
Pay range- $24 to $26.50 per hour
