Job Description
Licensed Practical Nurse
- The Blick Center is looking for a caring and dedicated full-time Licensed Practical Nurse (LPN) for their outpatient center and residential programs.
- The Blick Center provides services to individuals with developmental disabilities and/or a mental health diagnosis.
- No evenings or weekends
- Provide healthcare, support, and coordination of care
- Work closely with RN and direct care staff
- Assist in medication administration/health-related activity training to staff
- Document all nursing functions in the electronic medical record
- Relay pertinent health information to team members
- Assist the psychiatrist, primary care physician, and other health professionals, as needed.
- Attends program admission conferences, annual team meetings, and case reviews as needed
- Share on-call responsibilities (phone triage)
- Graduate from an accredited School of Practical Nursing with current licensure as a Licensed Practical Nurse in the state of Ohio.
- Minimum of two years nursing experience is required.
- Strong communication skills and interpersonal skills.
