- 12-hour shifts; 9am-9pm; Monday, Tuesday, Thursday
- One on one private care for disabled client
- Client is independent for most activities of daily living
- Transport of client required in clients auto
- Occasional assistance with catheter care
- Daily assistance with bowel program
- Meal prep and general caregiving assistance required
- Perform care as designed by clients individual Plan of Care and within scope of states nurse practice act
- Monitor patients condition and notify appropriate personnel of any changes
- Providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes
- Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
- Conduct ongoing assessments
- Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers
- Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report, and record adverse reactions to medications and/or treatment to the registered nurse
- Perform wound care and recognize the need for and institute emergency measures in a rapid and appropriate manner
- Follow principles of infection control and Universal Precautions
- Treat clients, family, and staff with courtesy and respect
- Current unrestricted LVN license in state of Texas
- 1+ years of experience on your license required
- 1+ years' Home Care experience preferred
- Reliable transportation / valid drivers license / auto insurance
- Current CPR/ BLS
- Negative TB test
