The Social Worker provides coordination of care in the home to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family for Hospice and Home Health services.
Requirements
ESSENTIAL DUTIES
- Conduct initial assessments within the time frame allowed by regulatory requirements and/or within timeframe services are requested.
- Develop and implements a plan of care that specifies:
- A statement of the client's psychological, social, emotional and spiritual needs;
- A Care Plan that address(es) each;
- Interventions; and,
- Participate in Intakes/Informationals/and Discharge Planning as needed.
- Creates continuity of care across all healthcare providers involved in patient's care by accurate, timely, and clear documentation of IDG notes. Provides information reflecting patient/family goals and interventions.
- Maintains positive inter-personal relationship with co-workers and other members of the health care team and the organization. Exhibited by timely and appropriate communication with the physician and other health care team members regarding the client's needs or progress. Obtains physician orders as needed, in accordance with policies and procedures.
- Creates and supports a safe environment for the mental and physical wellbeing of patients.

QUALIFICATIONS
- Current Kansas Licensed Bachelor Social Worker required, Kansas Licensed Master Social Worker strongly preferred.
- Graduate of accredited college/university with a Master's or Bachelor's degree in Social Work.
- Completion of one-year experience in a healthcare setting.
- No sanctions by the Kansas Behavioral Science Regulatory Board (BSRB).
- Valid driver's license as travel is required.
- BLS Certification
- Provide Proof of Auto Insurance.

- Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
- Understand and follows safety and infection control policies and practices.
WORKING CONDITIONS
While performing the duties of this Job, the employee is regularly required to sit and/or stand; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, peripheral vision, depth perception and ability to adjust focus.
