PROGRAM OVERVIEW:
The Bridge is a recognized leader in developing supportive housing for adults with serious behavioral health challenges; we house 1,500 people in 26 residential programs and 500+ scatter-site apartments including permanent supportive housing and two Safe Havens. Our housing specifically serves individuals with a serious mental illness and/or substance use disorder, including the formerly homeless, young adults, seniors, and veterans. Case management, advocacy, entitlement assistance, and therapeutic activities are provided to all tenants. Bridge residences offer a range of on-site services that are appropriate to the level of care and special needs of each resident.
THE POSITION
The role of the Licensed Practical Nurse (LPN) is to support adults diagnosed with serious mental illness (SMI) and medical needs living in Bridge supportive housing. The LPN will serve as part of a mobile team, engaging clients with an emphasis on community outreach and home visits within supportive housing. The selected LPN will provide psychoeducation, preventive medical screenings, short term medical support, escorts to important appointments, and support to recognize and manage chronic and acute medical conditions.
Utilizing a person-centered approach, the Licensed Registered Nurse will collaborate with a team of medical and human services professionals to improve the overall health and quality of life of our residents through the promotion of physical and mental health integration.
KEY RESPONSIBILITIES:
· Establishes supportive relationships with residents.
· Conducts health screenings and provides linkage to primary and specialty care.
· Perform assessments for health concerns.
· Educate residents about health maintenance and disease prevention.
· Completes progress notes and assessments in the electronic health record within a timely manner.
· Facilitate coordinated care for specialized or high need cases.
This is a Monday-Friday, 9am-5pm position that requires travel to Manhattan, Brooklyn and the Bronx.
REQUIRED/PREFERRED QUALIFICATIONS:
· Licensure as a Practical Nurse
· Experience working with adults diagnosed with mental illness and/or substance use disorders.
· Computer proficiency with Microsoft Office (Word, Excel, Outlook) and experience documenting notes within an electronic record systems
· Fluency in Spanish or another language a plus, but not required.
