- M – F, between the hours of 8:30 am – 6 pm (some flexibility required to attend evening events)

JOB DESCRIPTION: The Licensed Practical Nurse (LPN) is a member of the integrated clinical team and provides general medical care and treatment to patients in The Neighborhood Center Outreach Clinic. The LPN, acting within her/his professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of her/his care. The LPN will demonstrate safe, clinical decision-making and expert care for patients within the practice. The LPN will work collaboratively with the multi-disciplinary clinic team to meet the needs of patients, supporting the agency policy and procedures. The LPN will build effective working relationships with other health care professionals to deliver excellent primary health care services within the clinics.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Provide initial health assessments for all new clinic clients and annual follow-up assessments.
- Promotes patients’ health by developing day to day management and long term planning of the patient care area; directing and developing staff; collaborating with physicians and multidisciplinary professional staff; providing support for patients and their families.
- Coordinate care for medical issues, including all post admission medical testing on site or by referral to external medical providers, arranging for physical examinations of clients who have not had one within one year prior to program admission, scheduling appointments, and liaising with medical providers to obtain records.
- Acts as Health Coordinator for the program providing health education, risk reduction, counseling and referral service to all clients.
- If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR.
- Experience understanding various evidence-based models and theories pertaining to substance use and co-occurring disorders, psycho-education, motivational interviewing and recovery.
- Psycho-education about medical issues to all clinic clients, individually and in groups.
- Complete all required documentation, on time and accurately. This includes but is not limited to, progress notes, health screening assessments and care plans.
- Ensure client engagement in health and wellness practices, as well as chronic disease management in order to reduce hospitalizations and improve quality of life.
- Responsible for adhering to all standards and guidelines established by applicable regulatory authorities.
- Maintains self in a professional matter at all times, adhering to The Neighborhood Center, Inc.’s policies and procedures and in keeping with the Agency’s Mission Statement.
- Performs other duties as requested by Program Director or Agency Administration.
REQUIRED KNOWLEDGE and SKILLS
- Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, substance use disorders, serious mental illness and recovery. Extensive knowledge of substance use, mental health, family support services, and community resources.
- Use of Motivational Interviewing, recovery and trauma-informed approaches when delivering services.
- Experience understanding family systems based on diversity of cultural and ethnic backgrounds.
- Must be able to perform duties in an independent and efficient manner. Familiarity with and ability to use an Electronic Health Record system.
- The Licensed Practical Nurse is expected to practice in a recovery oriented, person–centered manner.
REQUIRED QUALIFICATIONS
- Must possess a current New York State Licensed Practical Nurse (LPN) license.
- Knowledge and experience in behavioral healthcare procedures preferred.
- Experience in health counseling/advising or CASAC certification.
- Experience and proven skill in positive interpersonal and human relations
- Experience in decision-making and problem solving skills.
- Demonstrated effective oral and written communication skills.
