JOB SUMMARY:
A Licensed Practical Nurse (LPN) is responsible for providing basic medical attention to patients and assessing their wellbeing as part of a larger medical team. Their duties include checking vital signs, recording a patient’s medical history and assisting patients with hygiene. LPNs work under the direct supervision of a more experienced Registered Nurse and record all patient interactions to develop thorough documentation about their care. They help prepare patients for procedures and assist with routine recovery care after initial treatment. LPNs usually work with several patients at once and administer care according to a schedule, upholding medical routines and procedures.
ESSENTIAL JOB FUNCTIONS/DUTIES:
● Assesses and triages immediate health concerns telephonically.
● Provides concise, appropriate, and safe nursing guidance.
● Identifies problems or gaps in care, offering opportunity for intervention, and ER diversion as appropriate.
● Communicates with patients, and respective members of the healthcare team regularly.
● Engages with fellow teammates, and leadership via multiple communication platforms.
● Provides health education and coaches consumers on available care options.
● Documents all patient interactions via multiple EMR - nextGen and tracking platforms.
● Special projects, initiatives, and other job duties as assigned.
QUALIFICATIONS:
● Current unrestricted LPN licensure in applicable
● Licensure in LPN for New York State
● Good standing LPN licensure
● Access to high-speed internet.
● Hard-wired via ethernet cable to the internet.
● Dedicated, private, distraction free space in home for home office.
● Mandatory attendance to training
● Full Time hour (40 hours per week - 8 hours per shift)
● Interview on camera
● 2+ years’ experience of Nursing
● Pediatric/OB experience for those specific roles; Peds for PCP offices; Internal Medicine Experience for specific roles
● Triage experience
● Proficient level of experience with Microsoft Office applications, and solid technical aptitude, ability to navigate through several system simultaneously
● Highly organized, self-directed worker with an ability to function in a high volume, fast-paced environment.
● Demonstrated solid verbal and written communication skills.
EDUCATION REQUIREMENTS:
● Bachelors Degree
