SalaryAmerican Hearts and Home is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in providing exceptional home care services to our clients. Job Responsibilities:
- Provide comprehensive nursing care to patients in their homes according to individualized care plans.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patient's vital signs and condition, and report any changes to the supervising Registered Nurse (RN) or physician.
- Educate patients and their families on disease management, medication administration, and proper care techniques.
- Maintain accurate and up-to-date patient records and documentation.
- Collaborate with other healthcare professionals to ensure coordinated and effective care delivery.
- Adhere to all relevant nursing standards, regulations, and ethical codes of conduct.
Qualifications:
- Valid LPN license in the state of GA
- Minimum of 2 years of experience in nursing, preferably in home care or a similar setting.
- CPR certification required.
- TB Test required
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Reliable transportation and willingness to travel to patients' homes within the designated area.
- Flexibility to work evenings, weekends, and holidays as needed.
