Job Description:
- Utilizes the multidisciplinary assessment to participate in the development of a comprehensive individualized service/recover plan.
- Thoroughly completes and submits appropriate reporting forms to UM when due.
- Completes all necessary documentation according to policy and procedure
- ensures individuals meet eligibility criteria or refers to appropriate provider in the community.
- Assists communication during scheduled doctors visits and assists with patient medications.
- Responsible for keeping all charts in compliance to set standards.
- Link and serve as advocate with other agencies to garner needed resources for individuals.
- Provides assistance and support in crisis situations.
- Ensures individuals are seen per utilization guidelines and per the designated frequency as stated on the treatment plan.
- Actively participates in multidisciplinary treatment/recovery team meetings.
- Maintains therapeutic professional boundaries when working with individuals and families.

Requirements:
- Licensed LPN or RN as per the State of Georgia
- 1 year experience working with Mental Health or Substance Abuse
- Clean 7 year motor vehicle report
- A pre-employment drug screen, random drug screen, and state & federal criminal background Check will be required of selected applicant.
