Licensed Practical or Registered Nurse Out Patient

Job Description:

• Utilizes the multidisciplinary assessment to participate in the development of a comprehensive individualized service/recover plan.
• Thoroughly completes and submits appropriate reporting forms to UM when due.
• Completes all necessary documentation according to policy and procedure
• ensures individuals meet eligibility criteria or refers to appropriate provider in the community.
• Assists communication during scheduled doctors visits and assists with patient medications.
• Responsible for keeping all charts in compliance to set standards.
• Link and serve as advocate with other agencies to garner needed resources for individuals.
• Provides assistance and support in crisis situations.
• Ensures individuals are seen per utilization guidelines and per the designated frequency as stated on the treatment plan.
• Actively participates in multidisciplinary treatment/recovery team meetings.
• Maintains therapeutic professional boundaries when working with individuals and families.

Requirements:

• Licensed LPN or RN as per the State of Georgia
• 1 year experience working with Mental Health or Substance Abuse
• Clean 7 year motor vehicle report
• A pre-employment drug screen, random drug screen, and state & federal criminal background Check will be required of selected applicant.
