Framingham
Wayside Youth and Family Support Network is looking to hire two full time LPNs (one 40 hour, one 30 hour) for the Youth Crisis Community Stabilization Program. The LPN is responsible for the oversight and implementation of the medical, health and medication services of individuals at the program, providing medical and health services delivery to all youth. The LPN is responsible for medication administration, reconciliation, obtaining/verifying orders, transcription, and other collateral contact as necessary. The LPN will perform Medical Risk Assessments within 12 hours of a youth’s admission, and makes all necessary recommendations for medical care to the Nursing Supervisor. In addition, the LPN will provide emergency triage, assess youth health and/or injuries for the Campus when necessary, similar to a school nurse position. Works closely with the Nursing Supervisor and Program Administrators and ensures youth conditions are medically managed for the duration of their stay. Triages medical requests from Program Administrators and communicates outcome to staff as appropriate daily. Maintains communication with youth’s legal guardian for all medical issues on a continual basis, as well as documents any encounters in nursing notes.
Salary Range: $30.00-35.00/hr. Salary for this position ranges from the “base” rate of pay (as listed above)—for someone with the minimum required credentials. Adjustments up from the base rate of pay could include such factors as years of related experience externally, educational background, years at another position within Wayside, needed language fluency, and/or particular areas of expertise. *****2,000.00 sign on bonus*****
What You Will Do:
- Administer medication to youth in the program.
- Collaborate with the program(s) and outside collaterals to ensure that the proper medication is being transcribed and administered to our youth
- Assist with medication transcription and consents as well as the medical portion of intakes & discharges.
- Provide nursing assessments and attend rounds to contribute to care planning.
- Collaborate with programs for nursing needs including health education/awareness groups. Provide education to staff and guardians on conditions/ medications.
- Be available for campus medical/medication issues, including assistance with psychiatry.
- Triage nursing needs and make appropriate referrals. Assist staff in training and troubleshooting medication distribution issues.
- Coordinate appointments with outside collaterals if necessary.
- Document all medical needs, encounters and communication in appropriate locations including EMR.
- Maintain a professional relationship with collaterals and guardians.
- Demonstrates high quality customer service (i.e. ability to connect with others, approachability, being respectful to others, able to communicate with others at all levels, strong client engagement skills, etc.).
- Demonstrates good clinical knowledge

- Active LPN license.
- At least 1 year of experience.
