Location: Adult and Children Residential Services.
Position Type: Full-Time On-Site
Work Schedule: Flexible Schedule; Monday-Friday
· (7am-3pm, 8am-4pm, 9am-5pm)
· No weekends or holiday
Responsibilities:
· Participate in written nursing plans of care for caseload with RN Supervisor.
· Maintain medical record for caseload.
· Ensure all medications are administered and stored as necessary.
· Prepare and review monthly MAR’s and medications.
· Ensure medical follow through on all medical appointments, including any new orders and follow appointments needed.
· Liaison with pharmacy in ensuring medications are provided to residence.
· Ensures compliance with OPWDD regulatory standards.
· Reviews Pharmacare and nutritional reports.

Qualifications:
· N.Y. State Licensed Practical Nurse License required
· N.Y. State Drivers License preferred
· Knowledge of OPWDD Regulations, preferred
· Knowledge of OMRDD Regulations preferred.
· Excellent communication and people skills.
· Ability to work independently and as part of a multidisciplinary team.
· CPR and First Aid certifications are preferred.
· Strong critical thinking and critical thinking skills.
