Responsibilities
LPN Job Summary The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. LPN Job Functions

• Provides direct patient care within the established Plan of Care and as defined in the State Nurse Practice Act.
• Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers. Assists team members as needed to ensure that this level of service is consistently provided.
• Implements planed interventions to meet the physical, psychological, social and environmental needs of the patient.
• Evaluates the patient's responses to the approved plan of treatment.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of hospice services, appropriately reporting any identified needs to the interdisciplinary group.
• Participates in Continuous Care as requested.
• Participates in clinical on-call program.
• Attends Interdisciplinary Group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement.
• Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
• Performs other duties as assigned by the registered nurse.

Qualifications
LPN Position Qualifications

• Graduate of an accredited Practical Nurse or vocational nursing program
• Current license to practice in the state of practice
• Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience
• Current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
• Demonstrates excellent observation, problem solving, verbal and written communications; skills per competency checklist
• Shows ability to organize and prioritize workload Independently
• Works indoors in patient homes

Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. This position requires a minimum of 8 education hours. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
