Must Haves:
- Bilingual English/Spanish fluency.
- Master's degree in social work, or comparable behavioral science field, from an accredited institution, or bachelor's degree with 3 years of employment experience that demonstrates supervisory and case management experience.
- Must demonstrate a sincere commitment to the service and advocacy for youth and families
- A valid professional license as required and acceptable by state or contract regulations.
- Driving personal vehicle in the performance of this job is required. Must have a valid driver's license, vehicle in good working order, and insurance with $100,000/$300,000 limits.
- Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
Day-to-Day:
The Lead Case Manager is responsible for coordinating case management for minors in the assigned program, training new Case Managers, and supervising the work of other Case Managers in assessing the needs of minors in care, developing Individual Service Plans, screening for human trafficking concerns, coordinating safe and timely release services, and other case management services
- Perform all work in a manner consistent with the organization's Mission, Values and Philosophies.
- Provide supervision, mentoring, case consultation, and support to the case management team on a daily basis.
- Coordination of safe and timely release services in collaboration with the cross-functional service delivery team.
- Travel as needed, to provide and support the provision of services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
- Complete all required training and ensure the case management team complete all required training in a timely manner.
- Establish and maintain a strong relationship with assigned foster parents.
- Support and assist program staff in locating and assessing potential sponsors, gathering their information, and assessing whether the potential sponsor is suitable.
- Guide and support program staff safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for children served.
- Participate in the Continuous Quality Improvement activities on a monthly/quarterly basis.
- Actively participate in the regular supervision process.
