Job description

Job Summary:
The Lead Case Manager is responsible for overseeing the Case Managers and providing guidance and supervision. Their role is to train Case Managers on policies around assessments and programming tools in the long-term foster care model. The Lead Case Manager will act as the check and balance for all report submissions to regulatory oversight, ensuring that the program remains in compliance with contractual agreement. They will lead staffing calls in relation to expediting permanency for clients within the UC program. In addition, the Lead Case Manager will carry a small caseload depending on program capacity. This position requires proficiency in oral and written communication in Spanish and English.
Essential Job Functions:
  • Promotes JCCA and the UC mission, programs, goals and accomplishments
  • Supervises all Case Management staff within the UC-LTFC program
  • Provides guided supervision and monitoring of all case management activities including documentation in same in all required systems
  • Assign new intakes to Case Managers ensuring that a balance of Basic and Therapeutic cases are maintained.
  • Coordinate legal activities with LSP for all cases in the program
  • Coordinate safe and timely release services of children in the UC programs
  • Ensure that all foster homes are monitored monthly and that Case Managers are providing all needed services
  • Work jointly with Lead Clinician around cases that require a multidisciplinary engagement
  • Collaborate with staff in creating and facilitating recreational activities
  • Train new Case Managers
  • Lead monthly staffing with Case Coordinator and quarterly staffing with FFS (Federal Field Specialist)
  • Complete progress notes and SIRs as needed
  • Complete and follow-up with Educational Planning and school visits as needed
  • Oversee search of viable sponsors and follow-up as needed
  • Accompany UC to Immigration Court and facilitate meeting with Immigration Attorney as needed
  • Liaise with Child Advocate
  • Complete ISP (Individual Service Plan), Case Reviews, Risk Assessments and UC Assessments as needed
  • Develop Behavior Modification Plans with UC and foster parent as needed
  • Provide on Call Support as required or schedule

Education and Experience Required:
  • Bachelor's Degree in Social or Behavioral Science; Master's degree preferred
  • Three years (3) progressive experience in Human Service or Social Sciences that demonstrate some supervisory and case management experience
  • Must have knowledge of trauma-based practices working with youth
  • Bilingual English/Spanish is REQUIRED

Physical Requirements:
  • Repetitive movements of hands and fingers (typing is required)
  • Ability to stand or walk long distances
  • Visual acuity to view documents on a computer screen for long periods of time
  • Typical office environment
  • Flexibility to work evenings and or weekends
  • Flexibility to travel out of state with minor if needed
