Full time, 36 hours per week
$15.00/hour minimum

Job Overview:
The Care Coordinator Lead oversees the Care Coordinator department and ensures the operations of the department are utilized throughout the organization.
Duties:

• Supports the development and implementation of the Care Coordinator department.
• Responsible for department's operational excellence and ensures that the department delivers quality services in accordance with applicable policies, procedures, and professional standards.
• Supervises Care Coordinator positions including disciplinary, time keeping, recruitment and employee relation issues.
• Assist in providing outreach and Medicaid enrollment within the community.
• Assists patients with applications to Medicaid, which also induces temporary Medicaid, presumptive Medicaid, disability Medicaid, pregnancy Medicaid, and verifies coverage.
• Interviews, orients, and continually mentors new hires for department with assistance of HR department.
• Prepare grant reporting.
• Conduct team meetings with department.
• Conduct trainings with other departments on topics related to functions of Care Coordination.
• Provide partial oversight of Show Me Healthy Women program.
• Provide assistance to coordinate organization-wide quality improvement activities to comply with Joint Commission and other external agencies reporting requirements.

Requirements
Preferred Qualifications:

• Management experience in healthcare setting.
• Associate Degree.
• Minimum of 1 year Clinic experience in case management or comparable position.
