Procare LTC Pharmacy, a division of Clarest Health, is seeking a Full Time IV Pharmacist for our Cheshire, CT location. The IV Pharmacist is responsible for providing consistent and ongoing monitoring of all infusion patients, including assessment of appropriateness of therapy, high risk medications, allergies, and other indications or interactions. In this high impact position, you will collaborate with nurses, providers, and internal departments to deliver excellent care. The IV pharmacist will lead a team of IV technicians to efficiently dispense daily IV medications, while maintaining up to date policies, procedures, and workflows for the IV Department to ensure regulatory compliance.
RESPONSIBILITIES
Act as Designated Pharmacist, per CT regulations, responsible for overseeing the compounding of sterile pharmaceuticals and the application of the USP chapters, as said chapters pertain to sterile compounding.
Complete approved Designated Pharmacist educational course per CT regulations upon assuming the IV pharmacist position.
Ensure compliance with all pharmacy laws and regulations, and sterile compounding guidelines, maintaining up to date policies, procedures, and workflows.
Directly supervise IV technician team (approximately 4 reports).
Maintain all required IV documentation and clinical monitoring.
Demonstrate thorough knowledge of prescriber/facility notification process for drug interaction, allergy, and other adverse patient safety situations.
Perform accurate dose calculations based on medication, patient condition, volume, and administration rates. Program infusion pumps as needed.
Have a deep understanding of Long Term Care Pharmacy and IV administration in Skilled Nursing Facilities.
Perform final review of sterile and non-sterile compounded medications for dispensing.
Compound IV medications in the event IV technicians are not available.
Answer drug-related questions as related to IV therapy and management in a timely manner.
Communicate with facility consultant pharmacists, clinical staff pharmacists, and facility representatives regarding IV patient management.
Orient, provide competency assessment and ongoing training for all fulfillment pharmacists and IV technicians. This includes making certain that all pharmacists and technicians rotating into the IV room have completed Infusion Training Program.
Excellent understanding of the GCPG, formulary initiatives, and clinical programs.
Maintain a current knowledge of infusion drugs and therapies and aseptic technique, as well as infusion devices, products, and supplies.
Attend continuing education seminars to maintain clinical pharmacy skills.
On-call duties as required.
Varied shift.
Ability to adhere to Clarests Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources, or the Compliance Officer.
Qualifications
IV Experience preferred
Supervisory experience preferred
Current Pharmacist license active and in good standing in the State of practice required
Bachelor of Science in Pharmacy or higher
