Responsibilities:
- Provide high-quality and compassionate care to patients per their individualized care plan
- Works collaboratively with the Hospice Home team in achieving patient care goals
- Maintain comfort, privacy, and dignity of our patients
- Promptly reporting pertinent information to a member of the leadership team.
Requirements:
- Current CNA listing with NC Home Health Aide Registry
- Must have reliable transportation
- Minimum of 1 year experience
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
