Job Summary:
The Infusion Patient Care Navigator Lead is responsible for assisting nurses to provide outreach efforts to patients needing care coordination for infusion treatments. The Lead acts as the point person for daily questions and provides education, traning, and mentorship to team members. This role will utilize the established model of care management. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Coordinates onboarding and off-boarding of clinic staff and provides education.
Schedules clinic staff assignments to ensure adequate coverage and equal distribution of work and effort.
Establishes course of action for self and others to ensure that work is completed effectively and efficiently.
Serves as a role model and mentor by demonstrating exceptional interpersonal communication and collaboration skills in the management of complex patient care situations.
Uses effective methods to help build a cohesive team and facilitates the completion of team goals.
Provides timely guidance and feedback to help others strengthen specific knowledge/skill areas needed to develop, accomplish a task, or solve a problem.
Ensures compliance with accreditation and regulatory bodies within area of responsibility.
Works with minimal supervision. Resolves office problems and facilitates solutions, confers with manager to resolve more complex issues, assists in training of new employees
Performs data entry of in-patient or procedural professional charges as assigned.
Collects time of service payments, posts payments in EPIC, and reconciles end-of-day receipts.
Takes accurate, clear and concise messages that provide sufficient information for the clinical staff and/or receiving party to effectively handle the issue/request.
Schedules, cancels and reschedules patient appointments, procedures, and surgeries.
Supports access by utilizing the wait list and filling appointment slots created by cancellations and no shows.
Greets and acknowledges patients at the front desk, checks patients in and out of the clinic, and performs practice-designated activities to support new patients.
Accesses EPIC to retrieve referrals, orders and other documentation and contacts various outside agencies to obtain referrals and other documentation.
Manages infusion templates and assists team with scheduling overrides.
Coordinates onboarding and training of infusion clinic coordinators.
Schedules infusion clinic coordinators assignments to ensure adequate coverage and equal distribution of work and effort.
Provides timely guidance and feedback to help others strengthen specific knowledge/skill areas needed to develop, accomplish a task, or solve a problem.
Monitors current workflows and suggests improvement when needed.
Other duties as assigned.

Required Job Qualifications:
High school diploma/GED required.
Previous experience in an ambulatory setting required.
At least two years of related experience required.
Ability to provide mentorship and training to other team members.
One year experience using electronic medical records.
Strong written and verbal communication skills.
Proficient in the Microsoft Office Suite.
Exceptional interpersonal skills.
Strong ability to multi-task in a large centralized scheduling center.
Ability to function independently, to organize, and be self-motivated.
Preferred Job Qualifications:
Associates or Bachelor s degree.
Bilingual highly desirable
