Job description

The position of Implementation Operations Manager is a field-based role requiring up to 75% travel time. The Implementation Operations Manager is responsible for evaluating potential nursing homes for their Operational capabilities and readiness to execute, advise on findings and deliver the LHP Medicare-Advantage (MA) Institutional Special Needs Plans (ISNP) operational processes and support the clinical model of care. Due to an expected high volume of new building launches, the Implementation Manager will assist multiple markets, with a high level of travel required.

Key Tasks and Responsibilities:*

  • Regular scheduled travel for assigned SNFs, visiting each at least weekly and ensuring ongoing communication with all relevant SNF leaders and LHP leaders.
  • Arrange evaluations at identified facilities. Include appropriate Facility staff, and corresponding LHP representatives.
  • Develop a plan with facility leaders for clinical, operations and enrollment orientation and training requirements, EMR training, and to provide a workspace for Longevity Health Plan Nurse Practitioner.
  • Arrange and attend additional weekly next-steps meetings with Nursing Facility and Longevity Health Plan leadership as necessary, in conjunction with LHP team.
  • Track and manage all pre-launch and post launch clinical activities, report on progress, issues, barriers to local team to ensure on-time launch.
  • Assist the LHP leadership with other clinical and operational activities, whether pre or post launch, to ensure optimal relationship building which supports continued growth and well-managed ISNP outcomes.
  • Ensure consistent adherence to Longevity Health Plan policies, procedures, compliance program, CMS requirements, and any state and federal applicable requirements.

Licensure:*

  • Valid state driver's license with a good driving record and proof of automobile insurance required.
  • Auto liability insurance is required with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage with combined single limit of $300,000. You will be responsible for providing this proof of coverage on an annual basis. Should you not have this level of coverage today, we will pay the delta to increase your coverage by increasing your base salary offer for that delta.

Education and Training:*

  • Bachelor’s degree or equivalent required. Master’s degree a plus.

Knowledge and Experience:*

  • Ability to travel frequently with reasonable notice as needed. Travel time up to 75%.
  • Experience in working in SNF, with an ISNP background preferred.
  • Experience in managed care, with value-based payment models in the nursing home/long term care environment and understand the value drivers for ISNPs.
  • Project management experience strongly desired.
