PROGRAM: Intensive Behavioral Health Services (IBHS)
POSITION FUNCTION:
The Care Coordinator engages youth, families, and other significant persons involved in the youth's treatment in a collaborative relationship to promote positive outcomes, assess youth and family social determinants of health, and provide supports to address identified physical and behavioral needs. The Care Coordinator assures that children’s and families' needs are coordinated among agency departments, outside agencies, and funding sources and that they are connected to any needed resources. The Care Coordinator is responsible for the day-to-day operations of the school or schools to which they are assigned, including responding to school needs, participating in school meetings, processing intakes, and making staff assignments.
EDUCATION/EXPERIENCE REQUIREMENTS:
- Bachelor's degree in psychology, education, or a related field
- 1+ years of case management or counseling experience with children and families
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to establish positive relationships with school officials, youth and families, and all members of the child-serving system
- Ability to effectively collect information and respond to questions from varied groups of children/families and the child-serving system
- Ability to communicate and interact with all team members
- Working knowledge of CASSP principles
- Computer literacy including pertinent software programs
- Ability to accurately calculate numbers (addition, subtraction, percentages, etc.)
- Familiarity with child-serving systems (e.g., education, juvenile justice, child welfare, mental health, intellectual/developmental disability, drug and alcohol, health care, and vocational rehabilitation)
- Problem-solving abilities with attention to detail
- Ability to interact with people in varied settings
- Ability to work independently and cooperatively as part of a team
- Ability to define problems, collect data, establish facts, and draw valid conclusions
ESSENTIAL JOB FUNCTIONS:
Major highlights of the position requirements are noted below. Additional responsibilities may be assigned as required.
- Monitor and conduct all aspects and responsibilities related to intake for the IBHS program.
- Monitor and track youth throughout the treatment process.
- Participate in all school meetings.
- Assess needs and connect children and families to resources.
- Act as a resource for the treatment team in crisis situations.
- Provide direct service as a Behavioral Health Technician if needed.
Communication:
- Maintain contact with treatment team members including schools, families, clinicians, and other relevant parties.
- Notify all participants of children’s/families' meetings and appointments.
- Clearly and effectively communicate pertinent information to responsible parties.
- Advocate for children and families to meet their needs and accomplish treatment objectives.
- Develop and implement service coordination plans in conjunction with the treatment team.
Documentation:
- Assemble all necessary documentation for submission to the funding source.
- Maintain accurate tracking documents as required by the funding source.
- Document contacts in the electronic health record.
Planning:
- Schedule interagency meetings, psychological evaluations, and medication evaluations at times that are convenient for treatment team members.
- Schedule appointments and meetings in a timely fashion to ensure continuity of service.
- Work with families to anticipate and address any barriers to family participation in the treatment process.
TRAINING REQUIREMENTS:
- Complete mandated orientation.
- Comply with Agency Annual Training requirements.
WORK ENVIRONMENT AND PRIMARY WORK LOCATIONS:
- Primary work location - Assigned school(s) in Philadelphia
- Some work conducted at CVCA’s New Covenant Campus
- Schedule is generally Monday through Friday, 8:00 A.M. to 4:00 P.M.
- Must be available for all assigned activities outside normal working hours
- Requires some scheduling flexibility, including evening and weekend hours
