Hyperbaric chamber duties: Performs hyperbaric chamber crew chief, chamber operator and inside attendant duties.
2.22.2. Have a working knowledge of and be able to troubleshoot issues with the atmosphere conditions system, gas farm, chamber lighting system, nitrogen services, patient monitoring system (cameras), potable water and chamber toilet, chamber oxygen monitoring, patient entertainment system,
pressurization and depressurizations systems, fire suppression system chamber gas analysis system, mask breathing system, and automatic chamber control system.
2.22.3. Build, inspect, and maintain hyperbaric dive masks/hoods.
2.22.4. Develop working knowledge of emergency hyperbaric procedures.
2.22.5. Prepare documents, turns in and tracks equipment in need of repair.
2.22.6. Prepare, issue and maintain file copy of temporary hand receipts for borrowed equipment.
2.22.7. Prepare and maintain hyperbaric diver files.
2.22.8 Once fully trained, conducts training for hyperbaric divers.
2.22.9. Conduct training for chamber crew positions (Chamber Operator, Crew Chief, and Inside Attendant).
2.22.10 Chamber operator duties include start-up, shut-down, and running the chamber. Chamber operator monitors patient's conditions during hyperbaric treatment dive and coordinates care with the inside attendant and hyperbaric physician.
2.22.11. Crew Chief assists the chamber operator during routine operations, and also assists the chamberoperator in the event of problems. Troubleshoots and corrects problems during the dive and between dives as necessary.
2.22.12. The inside attendant must meet and maintain standards for hyperbaric chamber duty IAW AFI 48-123, Medical Examinations and Standards, para 6.48.5., as determined by a USAF flight surgeon; must not be in a "duties not involving diving" status for longer than 90 days. The exception to this standard is pregnancy. The inside attendant will be certified with a minimum of three supervised training hyperbaric dives.
2.22.13. The technician should be expected to act as inside attendant at least once per week, depending upon patient chambers.
2.22.14. Inside attendant is inside the chamber during the treatment dive and monitors patients throughout the dive. The attendant will assist patients with equalizing pressure in their ears during the pressurization and depressurization. The attendant will assist with loading, unloading and positioning of patients in the chamber.
2.22.15. Attaches wound vacuum during dive, monitors glucose if necessary.
2.22.16. Prepares the procedure room/equipment, gathers and/or furnishes supplies, in an efficient and timely manner, properly prepares and positions the patient and effectively assists providers in performing procedures.
2.22.17. Directly performs or assists in the performance of a number of specialized minor nursing procedures, including, but not limited to minor wound debridement and dressing changes.
2.22.18. Prepares the procedure room and medical equipment for the procedure to be performed. Receives the patient and reviews his or her medical history.
2.22.19. Uses the nursing process and participates in making nursing assessments. Prepares and positions patient for the procedure and monitors vital signs.
2.22.20. Closely monitors the patient's condition throughout the procedure and reports and/or notes changes in patient's vital signs, symptoms, and general physical and emotional condition for inclusion in the patient care plan.
2.22.21. Performs procedures such as electrocardiograms (EKGs), urinary catheterizations, and suctioning of trachea.
2.22.22. Documents the procedure in the patient's chart, hard copy or digital, asrequired.
2.22.23. Monitors and evaluates patient response during exams, procedures, and treatments.
2.22.24. Collects and labels, urine, sputum, blood, and stool specimens.
2.22.25. Obtains wound and other cultures. Inserts and drains in-dwelling catheters, removes sutures, irrigates wounds, and performs venipunctures.
2.22.26. Adheres to established aseptic procedures to protect self, other staff members and patients from unnecessary exposure to infectious agents.
2.22.27. Cleans and prepares equipment for sterilization.
2.22.28. Disposes of infectious and hazardous waste materials in accordance with established procedure.
2.22.29. Assists in patient treatment process and disposition. Coordinates with medical and maintenance personnel on chamber equipment configuration and modification and patient hyperbaric treatment and hyperbaric research.
2.22.30. Calculates dosages and administers medications by oral, topical, intradermal, subcutaneous, and intramuscular routes.
2.22.31. Initiates intravenous fluids to include saline locks and monitors, regulates, and discontinues their usage.
2.22.32. Observes and promptly notifies nurse and/or physician of adverse physical and emotional reactions to medications. Exercises control over pharmaceuticals.
2.22.33. Documents administration of medications in accordance with established charting procedures.
2.22.34. Recognizes the nature of emergencies and provides first aid in accordance with emergency protocols.
2.22.35. Initiates and maintains cardiopulmonary resuscitation (CPR) measures if first on the scene until additional responders arrive.
2.22.36. Conducts patient and family teaching. Teaches patients and family members how patients should perform simple tests for themselves, e.g., blood sugar, hypertension or urine. Informs patients and family members about self-care. Reinforces and reiterates instruction previously presented by the physician or nurse. Provides input for patient care planning. 9 Skill set info 2.21.1. Licensed Vocational Nurse (LVN) with a minimum of 24 months clinical experience within the past 36 months. Experience in hyperbaric medicine and wound care is desirable, but not required.
2.21.2. The ideal technician will possess a certification as a Certified Hyperbaric Technician (CHT) from the National Board of Diving and Hyperbaric Medical Technology (NBDHMT) prior to performance start or hire date. However, if not certified as a CHT prior to performance start or hire date, the technician will obtain CHT certification within six (6) months of performance start or hire date. In the event that the technician does not obtain CHT certification within six months due to failure of the certifying examination a second attempt may be afforded. After an initial examination failure, the Hyperbaric Medicine Clinic Flight Commander/Medical Director, Safety Director, and NCOIC will make a recommendation to the contractor regarding continued employment based on the employee's ability to safely and competently perform their assigned duties. The NBDHMT requires a 6-month waiting period prior to a second examination attempt and the employee will reattempt the examination at the soonest allowed time. CHT certification requires completion of a 40 hour Undersea and HyperbaricMedicine Society (UHMS) introductory course; 480 hours of clinical experience; training in transcutaneous oxygen measurement (TCOM); and complete a certifying examination. The Undersea and Hyperbaric Medicine Society (UHMS) introductory course is not available through the government and will not be provided.The contractor shall pay for the UHMS course. The 40 hours for CHT certification can be included as part of the 480 hours of clinical experience. The technician shall complete the UHMS introductory course and the certifying examination at the contractor's expense. The technician can attain the 480 hours of clinical experience and TCOM experience during duty time at the MTF at the contractor's expense. Following certification, the technician shall complete 12 hours of continuing education biannually (twice each year). The technician shall maintain CHT certification. CHT testing can be provided by the government. The cost for initial certification and recertification is at the contractor's expense.
2.21.3. Technicians shall have and maintain certification in Basic Cardiac Life Support (BLS) for Healthcare Providers which must be obtained through the American Heart Association or a Red Crossapproved training program.
2.21.4. Technicians shall undergo a physical performed by a licensed provider, which must meet USAF dive standards as outlined in Air Force Instruction 48-123, para 6.24.8 The physical is the responsibility of the technician to obtain. Once the technician has an approved physical, the MTF will conduct a test of
pressure in the hyperbaric chamber at the MTF as the final basis for selection into the Hyperbaric Clinic.
