We are rapidly growing and have new positions open due to growth of our business and expanding programs. We are seeking a Bachelors prepared Social Worker (or someone trained in a related field) to work in the Truckee Area Staff will be working with low-income clients who reside in a special housing complex and those who are transitioning out of nursing facilities. HHCM is looking for a Part time Social Worker.
The ideal candidate has:
1. Excellent written and verbal communication skills
2. Understands and can provide a comprehensive social assessment of a client
3. Willingness and skills to work with clients, facility discharge planners, Social Services Directors and family members to ensure a smooth and transparent transition process
4. Enjoys working in an interdisciplinary work setting with Registered Nurses and clerical staff to perform transition coordination (we will provide training for CCT program)
5. Copy of Social Work Degree (Bachelors Required) BA in Social Science or related field such as gerontology, health services, drug or alcohol/mental health training.
6. Driver's License, reliable transportation, auto insurance, and CPR certificate, clear driving record (no DUI).
7. Must be willing to drive between Sacramento and Davis to make home visits to clients. (Personal Protective Equipment & Training is provided for all staff)
8. Ability to work with electronic medical health records and computer systems.
9. Bilingual in Spanish, Hmong, Mien, Russian, or other language would be preferred but not required.
Salary & Benefits
• Dependent on experience range is $20-24/hr.
