Position Description: Housing Program Manager - Clarksville
Position Type: Full Time/Non-Exempt
Supervisor: Associate Director of Medical Case Management
Location: Nashville, TN
As a Housing Program Manager at Nashville CARES, you will assist clients living in Montgomery, Houston, Humphreys, and Stewart counties with accessing housing and supportive services within the agency and throughout outside sources.
What You’ll Do
· Assess each client’s circumstances and connect them to appropriate services to meet their needs
· Assist clients with locating new housing or maintaining their current housing by facilitating access to CARES’ internal housing resources, as well as those in the community
· Oversee client eligibility for the HOPWA program and ensure only clients who are eligible are receiving HOPWA-funded services in the region
· Ensure clients on the Tenant-Based Rental Assistance program maintain their housing by meeting with them in their homes on a regular basis and completing all necessary paperwork to sustain their subsidy
· Work with clients to facilitate eligibility for the Ryan White program
· Coordinate closely with peers to ensure clients have access to all needed resources
· Meet regularly with clients in the office, in clients’ homes, and in the community to complete biopsychosocial assessments and assist with connection to medical, social, community, financial, and other needed services
· Develop and maintain relationships with community housing providers and landlords
· Assist your clients with identifying their own goals for housing and healthcare and then navigating the various systems involved in achieving those goals
· Participate in community meetings as a representative for the agency and your clients
· Complete all documentation in an appropriate manner and in a reasonable timeframe, including funder required paperwork, according to agency and community standards to ensure staff and clients do not encounter barriers
· Maintain appropriate boundaries with clients and with work

Minimum Requirements:
· Bachelor’s degree (preference for a degree in Social Work or other Human Services)
· Minimum of three years’ experience in community based social or health care services
· Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology
· Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work

Pay Range: $42,000-$45,000 annually
