Housing Intake Manager, Continuum of Care
The Housing Intake Manager, Continuum of Care (COC) position is dedicated to moving people experiencing homelessness into housing. This position will manage the process by which Thresholds fills vacancies on our Chicago scattered-site Permanent Supportive Housing contacts. The Manager will bring together a cross-departmental team to efficiently and effectively improve service delivery, balancing tenant experience with organizational needs. Position reports to Assistant Director, Housing Administration.
Key outcomes of the position include: decreasing the time from referral to housed; ensuring that internal process meets CoC expectations and is reflected in HMIS data; considering all processes from a participant-focused and trauma-informed care lens.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Manage all communication with AllChicago/Coordinated Entry about vacancies on Thresholds’ 400 units of scattered-site PSH in Chicago
- Request matches for vacant CoC units, screen referrals for eligibility according to HUD homeless definitions and within a trauma-informed care context
- Manage and refine referral pathway within Thresholds to reduce time between referral and housing, including Service Connection needs assessment, housing search, and move-in
- Understand the requirements of individual housing grants and confirm that referrals are eligible for the contract which they are matched
- Track referral progress in HMIS in accordance with AllChicago expectations
- Supervise Housing Locator and support landlord outreach as needed
- Ability to lead and facilitate presentations, trainings, etc
- Collects and manages tracking and other data on all aspects of the housing referral process
- Other duties as assigned
- Must have comprehensive knowledge of Microsoft Office products, including internet and email. Knowledge of or ability to learn other pertinent software products preferred. Intermediate to advanced Excel skills are required.
EDUCATION & EXPERIENCE
- Bachelors Degree in a related human services field with preferred 3 years of experience
- Relevant experience could include clinical outreach, case management and/or housing location with people experiencing homelessness, management of CoC or other government-funded programs related to ending homelessness, and/or other positions with permanent supportive housing providers or funders.
- Experience with HMIS preferred.
- Understanding of the target population to be served.
- Understanding of housing first and trauma-informed care practices
- Basic knowledge of Landlord/Tenant responsibilities.
- Conversational Spanish preferred.
SKILLS/CERTIFICATIONS
- Valid driver’s license required; routine access to a personal vehicle required.

- Competitive pay – Base salary: $49,000.00 - $61,000.00 Annual Salary
