What you will do:
Hospital Liaison is a professional representative of the Agency who functions as a health care educator and intermediary between the Agency and hospitals, facilities, health care professionals and the community. The purpose of this role is to facilitate an uncomplicated transition of patients to home care consistent with the mission and goals of the Agency.
You will:
- Assist in identifying appropriate patients through reviewing referrals, collaborating with hospital and case managers for patients to assist in securing appropriate Agency services and follow-up support.
- Helps to achieve and maintain continuity of patient care by planning and exchanging information with other health professionals within a hospital to home setting.
- Maintain relationships with Hospital, utilization review nurses, discharge planners, case managers, physicians and other health care professionals within a hospital.
- Provides appropriate education and marketing materials to patients, families, healthcare and hospital personnel in order to effectively promote utilization of Agency’s services.

Who you are
- High School Diploma is required, Associate degree or higher preferred.
- Preferred, but not required: Current, active license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners may be required based on Hospital assignment
- A minimum of 2 years healthcare services, sales or related industry experience
- Knowledge of hospital, physician, skilled nursing facility, case management needs
- Ability to effectively communicate Agency’s services with all levels of the healthcare delivery team and clients.
- Must possess excellent organizational skills and be detail oriented.
- Must demonstrate strong written and verbal communication skills and be able to interact effectively with health care professionals, patients, peers and management.
