Hospice Social Worker

California Hospice Care Center is seeking for a motivated and caring Social Worker to join our team.

Requirements:

• MSW preferred BSW required.
• Licensed Clinical Social Worker (LCSW)
• Minimum of one year social work experience in hospice
• Current driver's license
• Clean background and DMV driving record
• Maintain auto insurance coverage
• CPR certified

Responsibilities:

- Conduct assessments and evaluations to determine clients' needs and develop appropriate treatment plan.

- Provide individual and group counseling sessions to clients with behavioral health issues

- Collaborate with other professionals, such as doctors, nurses, and educators, to coordinate care for clients

Maintain accurate and up-to-date client records and documentation

- Assist clients in accessing community resources and support services

- Advocate for clients' rights and ensure their needs are met

- Understanding of hospice care principles and practices

Note: A valid social work license is required for this position.

Pay: $80.00 - $130.00 per hour

Expected hours: 10 – 20 per week
