Olive Branch Hospice is looking for a self-motivated and energetic Community Liaison to join our Team.
The Community Liaison (Sales Representative) will promote Olive Branch Hospice’s unique program and services in accordance with established policies and procedures. The Community Liaison is responsible for growing the census and the admission goals of the organization. The Hospice Community Liaison will participate in the development of a community relations and professional relations program to increase hospice awareness and recognition while identifying new referral sources and maintaining ongoing relationships with hospice partners, physicians, and referral sources.

RESPONSIBILITIES
· Proactively builds positive, solid working relationships with members of the medical community and partners.
· Promotes and positions Hospice's patients care service through regular visits in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities.
· Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing, referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
· Conducts initial meetings with the patient and family giving an overview of hospice service and philosophy and obtains consents for care.
· Assists with the development of information materials and packets.
· Coordinates special events for Community involvement.
· Assists with the coordination of public relations/media/advertising activities.
· Represents Olive Branch Hospice at local community events as requested.
· Assists with correspondence and agency communication.
· Maintains all necessary documentation and confidential files for regulatory compliance.
· Assists with special events, projects, and activities.
· Assists Volunteer Director with program development and volunteer retention.
QUALIFICATIONS
· College degree preferred. Additional experience may be substituted for educational requirements.
· Two years of experience in marketing/community relations in a health care related industry, hospice experience preferred.
· Proficient computer skills and familiarity with Excel, Power Point, & Word.
· Must possess excellent communication and customer service skills.
· Must be self-motivated and disciplined.
· Must possess excellent organizational and time management skills.
