Job Description
Purpose: A Hospice Registered Nurse provides skilled nursing care to hospice patients and their families in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Clinical Manager.
Essential Responsibilities:
1. Provides comprehensive nursing care for hospice patients, including pain and symptom management.
2. Conducts initial assessment of patient and their family, evaluates patient risk for preventive and rehabilitative nursing procedures, and regularly reevaluates the patient's nursing needs.
3. Develops and initiates a plan of care, makes revisions as necessary, and updates regularly.
4. Documents patient progress towards achieving clinical goals, plans care conferences for patients, and conducts discharge planning following agency policy.
5. Processes physician orders and notifies doctors of patient needs and condition changes. Completes certification/recertification orders and discharge summaries.
6. Collaborates with interdisciplinary care team and makes appropriate referrals to other healthcare professionals as needed.
7. Provides education and support to patients and family members to ensure safe home management and comfortable end-of-life care.
8. Participates in staff meetings, in-service programs, and adheres to established agency policies and procedures.
9. Conducts patient care conferences for assigned patients.
10. Participates in onboarding and mentoring new nursing personnel, as well as quality assessment and performance improvement activities.
11. Willing to work After Hours, weekends, and holidays as scheduled.
12. Completes necessary documentation and reports in a timely manner in accordance with agency guidelines.
13. Demonstrates professionalism, compassion, and respect in all interactions.
14. Performs other duties as assigned.
Requirements
Education and Experience Requirements:
- Graduate of an approved school of nursing and currently licensed as a Registered Nurse in the state(s) in which practicing.
- Two (2) years of professional nursing experience, preferably in hospice or home health care.
- Acceptance of philosophy and goals of accredited hospice program.
- Ability to exercise initiative and independent judgment.
Minimum Knowledge, Skills, and Abilities Requirements:
• Basic computer EMR skills
• Valid driver's license and reliable transportation
License, Registration, or Certification Requirements:
• Current RN license in state of practice
• CPR certification
• Hospice and Palliative Nursing Certification (HPNA) preferred
