Essential Duties and Responsibilities
- Exercise clinical judgement in combination with utilization of protocols to review clinical results, determine urgency, and summarize/route results to the provider
- Analyze clinical and results data, identify needs, patterns, and variances, and communicate findings to the provider
- Perform outbound phone calls to communicate results on behalf of the provider
- Prioritize and respond to incoming patient messages and test results
- Provide clinical support for result related questions/concerns and determine/facilitate appropriate next steps
- Assist with prioritizing and managing patient portal messages related to patient results
- Perform clinical data entry for patient results and quality metrics per protocol
- Educate patients, family, and others on health maintenance, disease prevention, and initiate education plan per protocol
- Collaborate with peers and colleagues to enhance professional nursing practice
- Maintain clear and compassionate communication with patients, family members, providers, and other external entities, representing the company in a professional and courteous manner in all interactions
- Demonstrate exceptional customer service by addressing all inquiries, concerns, and needs with empathy, professionalism, and a solutions-oriented mindset
- Adhere to all state and federal laws, including HIPAA
- Appropriately identify, acknowledge, escalate and/or address patient complaints, grievances, and concerns in a timely and effective manner
- Collaborate with the care team members to promote quality patient care, satisfaction, and outcomes
- Participate in quality improvements as assigned

- Graduate of approved or accredited nursing education program as a Licensed Practical Nurse (LPN)
- Active LPN or LVN license in the state where patients are located
- 2 years of experience as an LPN or LVN
- Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role
- Ability to react decisively and quickly in urgent and emergent situations
- Knowledge of medication effects, indications, and contraindications
- Demonstrated experience with multi-tasking
- Demonstrate proficiency in using electronic health records (EHR) system, call center technology, and Microsoft Office Suite
- Ability to communicate effectively with HarmonyCares providers and patient care teams
- Ability to communicate effectively with patients and their family members, and facility staff
- Knowledge of basic principles, practices and techniques in primary healthcare
- Ability to read and interpret written information, and write clearly and informatively
- Ability to speak clearly and persuasively in positive or negative situations, including listening and getting clarifications, and responding well to questions
- Working knowledge of healthcare industry
- Familiarity with Medicare and Medicaid insurances and programs
- Experience in Aprima (EHR)
