```Duties```
- Provide compassionate and quality care to hospice patients in their homes or healthcare facilities
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor and evaluate patient's physical and emotional well-being
- Collaborate with interdisciplinary team members to coordinate patient care
- Educate patients and their families on disease management and end-of-life care options
- Document patient assessments, interventions, and outcomes accurately and timely
- Maintain patient confidentiality and adhere to HIPAA regulations
- Provide emotional support and comfort to patients and their families

- Strong knowledge of physiology, anatomy, medical terminology, and medical documentation
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to provide compassionate care while maintaining professional boundaries
- Strong critical thinking and problem-solving abilities in assessing patient needs and developing appropriate care plans
- Proficient in using electronic medical records systems for documentation purposes
- Ability to work independently as well as collaboratively within a multidisciplinary team
- Attention to detail in accurately documenting patient information and following protocols
