Empyrean Hospice hires Licensed Practical/Vocational Nurses to provide direct patient care under the supervision of a registered nurse.

Salary or hourly rate listed is the base for this role; increase commensurate with experience.


Position Overview Licensed Practical/Vocational Nurse:

Represent Empyrean Hospice with the utmost professionalism and compassion

Provide direct patient care under the supervision of a registered nurse and as defined in State Nurse Practice Act, including specialized duties related to end of life care

Work collaboratively with the members of the interdisciplinary team to help meet positive patient care outcomes

Assess and provide patient and family/caregiver education and information pertinent to diagnosis and the plan of care.

Excellent observation, verbal, and written communication skills, organization and critical thinking skills, basic math skills; nursing skills per competency checklist

Understand the philosophy of hospice and the needs of the terminally ill

Complete, maintain, and submit accurate and relevant clinical notes regarding patient's condition and care given

Provide and maintain a safe environment for the patient and arrange equipment and other necessary items and services as needed

Ensure standards of ethical business and clinical practice are prioritized

Position Qualifications

Professionalism, compassion, empathy, and a desire to help others

Self-directing with the ability to work with little direct supervision, yet also as member of a complete interdisciplinary care team

Graduate of an accredited Practical Nurse or vocational nursing program

Three years nursing experience. Community health/hospice or medical/surgical experience is preferred

Currently licensed as an LPN/LVN in the State and complies with accepted professional standards and practice

Demonstrates good verbal and written communication, and organization skills.

Possesses and maintains current CPR certification

Able to lift, position and/or transfer patients, and able to lift supplies and equipment

Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order

Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
