Patient-Centered Care
By considering every facet of a patient's life, members of the Care Dimensions clinical team bring a holistic perspective to the care they give. Each patient is viewed as a whole person with physical, emotional, spiritual and social needs. We offer specialized clinical programs for patients with advanced dementia, cardiac and respiratory diseases. We Are There Wherever You Need Us
We provide care for patients wherever they call "home" - private residences, assisted living facilities, long-term care facilities, hospitals, group homes and veteran's facilities. Patients who need acute symptom management can be admitted to our 20-bed Kaplan Family Hospice House, which opened in 2005 as the first licensed inpatient hospice facility in Massachusetts.
Palliative Care for Advanced Illness
For patients who aren't ready for hospice, our palliative care program adds a layer of support for patients with serious illness by providing relief from pain and other symptoms; assisting families in making difficult medical decisions; coordinating care with the patient's physicians; and providing emotional and spiritual support.
Reaching Out to the Community with Education
The staff at Care Dimensions provides outstanding training and accredited continuing education opportunities to healthcare professionals. We also offer workshops and classes to members of the community who seek to better understand issues surrounding advanced illnesses and end-of-life decisions.
Helping Families Cope with Loss
Care Dimensions is widely recognized for its bereavement program - considered to be the most comprehensive in New England. We help honor the memory of those lost by providing grief support to 5,000 adults and children each year. We offer counseling services, workshops, support groups, bereavement camp, and crisis intervention at schools, businesses and community organizations.

Summary:
The Hospice Aide is a vital member of the interdisciplinary team. He/she is assigned to care for patients who require personal care services. The Hospice Aide reports to the Clinical Manager or Hospice House Manager, but receives direct supervision from the RN Case Manager or designee.
The Hospice Aide is responsible for providing and/or assisting the patient with personal care needs, ambulation, supplementary services including meal preparation, and other appropriate tasks as indicated by the RN Case Manager or designee. Charting on the appropriate agency forms is also required.
Qualifications:
High School Diploma or equivalent preferred.
Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services.
Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period.
In lieu of an HHA certificate, successful passing of Hospice Aide written test and skills assessment.
Experience in the field as a home health aide or certified nursing assistant is preferred.
Availability to work day, evening or night hours, including weekends and holidays as required.
Has a compassionate, warm and accepting attitude toward people.
Demonstrates a sympathetic, mature attitude in caring for the sick and disabled and an ability to deal with different ideas and value systems.
Ability to respect confidentiality of patients and families.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

Our 18-bed inpatient Care Dimensions Hospice House in Lincoln is OPEN and we're hiring! The Care Dimensions Hospice House will feature many of the same attributes that have made Kaplan House such a refuge for patients and families, including:
18 private suites with wheelchair-accessible bathrooms; two rooms will be equipped for pediatric patients
Tastefully decorated home-like rooms that encourage the display of personal items and photographs, and have window seats or openings to patios with views of the gardens and natural surroundings
Considerable space to accommodate reclining chairs and comfortable couches, allowing loved ones to stay overnight
A warm, welcoming atmosphere with fully-equipped kitchens, living rooms, chapel, library, children's areas, meeting room and counseling areas
A serene, tranquil environment with lovely gardens, pathways and views of the Cambridge Reservoir

Summary:

- High Integrity; Ability to apply good judgement
- Valid driver's license and auto liability insurance
- Ability to read, write, and speak the English language
- Experience as a Certified Nursing Assistant is preferred
