Under the direction of the Nurse Supervisor, the Certified Hospice Aide will perform assigned duties in the McCarthy Care Center in a safe and considerate manner as prescribed by the patient plan of care.
Description
- Maintains open communication with LPN/Nurse Supervisor regarding the health, safety and environment status of patients. Reports all changes in a timely manner.
- Provide physical, emotional, psychosocial, and spiritual care as part of the interdisciplinary team in the last phases of incurable disease so they may live as fully and comfortable as possible.
- Provide support with diversional activities to enhance quality of life for the patient.
- Respect the choices of the patient and the family concerning the patient's end of life care and plan of care.
- Provide a dignified comfortable end of life experience, as well as non-pharmacological comfort measures as identified by the patient plan of care.
- Report to nurse any signs and symptoms of changes in patient's status.
- Complies with State/Federal regulatory acts.
- Perform all skills with competency, as indicated on the skills checklist.
- Communicates with Nurse Supervisor regarding all scheduling concerns.
- Communicates with Nurse Supervisor regarding all concerns of possible patient abuse or neglect.
- Completes and submits required documentation of patient care according to plan of care.
- Establishes and maintains a professional and therapeutic relationship with patients/families.
- Performs only those tasks as prescribed by the Nurse supervisor on the Hospice Aide plan of care.
- Encourages patient in achieving an optimum level of independence by encouraging his/her participation in personal care and in interests and activities in the home, family and community.
- Provides or assists with post mortem care after pronouncement.
- Provides understanding and emotional support to patient and significant others.
- Consistently provides service excellence to all patients, family members, visitors, volunteers, and co-worker in a manner that reflects VNA's commitment to CARES: compassion, accountability, respect, excellence and service.
- All other duties as assigned.
Qualifications
- Possess and maintain certification as a certified home health aide or equivalent as approved by the agency.
- One year satisfactory employment history preferred.
- Experience in care of the elderly preferred.
- Satisfactory completion of Skills Assessment Test.
- Able to record messages and keep records.
- Complete hospice training provided by agency.
