Summary of Job Function: This position provides outreach services to individuals experiencing homelessness, with a focus on those individuals sleeping on the streets or in places not meant for human habitation. Engage clients experiencing homelessness into programs to assist with permanent housing. Will also provide assessment as needed and warm handoffs to ensure that timely services are initiated.

Essential Functions:
1. Will complete Intakes, Supportive Individual Counseling, and Targeted Case Management to individuals who are homeless and have been engaged into the program.
2. Actively seek out, identify, and build rapport with homeless individuals and families living on the street or in places not meant for habitation.
3. Be part of each communitys local housing prioritization team, or work diligently to aid in the development of prioritization for housing within the community.
4. Conduct assessments in accordance with local continuum of care policy.
5. Facilitate entrance into housing programs in the local continuum of care.
6. Assist clients in breaking the cycle of homelessness by moving them from the street to appropriate housing and accessing necessary social services resources using a housing first philosophy.
7. Work to identify local landlords to secure housing units, negotiable rates, and process rental assistance paperwork for payment.
8. Assist in efforts to secure housing for program participant by utilizing existing resources within the county. This will include but is not limited to assisting the program participant in filling out housing applications, and locating mainstream resources to assist with security deposits, and other housing assistance needs. 9. Coordinate and lead outreach efforts by Continuum of Care
10. Provide individualized consumer supports, using evidence based case management tools to assist consumer with developing a plan to address barriers facing them in maintaining and sustaining permanent housing.
11. All other duties as assigned
Job Specifications:
1. Education, Certification/License, and/or Experience
a. Bachelors Degree in a Human Services or Related field from a four-year college or university.
b. Drivers license and reliable transportation is required.
c. Obtain and maintain CPR/FA and CPI certifications.
2. Knowledge, Skills, and Abilities required: Must have knowledge of Continuum of Care policy and the ability to read and interpret documents, write routine reports and correspondence, speak effectively before groups of customer or employees of organization, problem solving ability. Must have the ability to add, subtract, multiply, and divide numeric figures.
