Experience and Education    
  • Undergraduate degree and one (1) to two (2) years as Speech Therapist is required.
  • Licensed in Cardio-Pulmonary or Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC)
  • Speech Therapy and valid Texas drivers license is required.
  • Preferred: Home health services      
Job Duties      
  • Evaluates patient's home environment and communicative status of patient. Prepares documentation that reflects identified problems or disability, interventions, communications, and patient responses and is supported by Plan of Care. Completes necessary forms and completes documentation that reflects compliance with agency policies. Develops plan of care in coordination with physician.
  • Bases plan of care on initial assessment, speech and language disorders, and medical diagnosis and completes in timely manner. Ensures plan of care is relevant to patient's current needs.
  • Outcomes are evaluated and suggestions for change made accordingly. Provides patient care and coordinates care with other disciplines and team members with assistance of Clinical Supervisor.
  • Prioritizes and evaluates patient care on ongoing basis. Assists patient in improving, correcting, and/or accepting his or her disability are taught to patient, family or caregiver, and health team members as necessary.
  • Assures that patient care is performed within speech pathology policies and procedures in accordance with rules, regulations, codes of ethics, guidelines, and codes in federal, state, and local laws concerning practice of speech therapy. Adheres to proper infection control, hazardous waste, HIV or HBV, etc. procedures in accordance with agency policy. 
  • Coordinates patient care with other disciplines and team members. Observes changes in patient's condition and reactions to treatment are reported to physician.
  • Participates in team conferences as required per agency policy and constructive and relevant information is provided. Gives daily reports to Clinical Supervisor regarding patient problems and changes to plan of care.
  • Documents care and interventions. 
  • Writes legible and complete documentation, and reflects agency policies. Submits written reports of patient's progress on appropriate forms to physician and agency at designated intervals of every 60 days and at discharge of patient. Participates in implementation of Quality Improvement Plan. Understands role in Quality Improvement Plan can be verbalized.
  • Completes Quality Improvement activities assigned by General Manager or Clinical Supervisor within given time frame. Maintains safe and healthy environment. Instructs patients on safety at home. Uses universal precautions. Maintains PPD and CPR per agency policy. Performs other duties as assigned.
