Job Description
Our dedicated, patient-focused and Medicare Certified Home Health agency is seeking a Home Health LVN Nurse Office Case Manager to join our growing team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients while collaborating with our patient facing field Nurses.
REQUIREMENTS:
- New Grads Welcome
- Valid CA Nursing license
- Candidates must also have excellent customer service
- Interpersonal skills
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Excellent computer skills,
- A working knowledge of Title 22 and CMS regulations.
***Experience with Electronic Medical Records and QA/QI Knowledge is a plus.

Responsibilities:
- Collaborate with Field Nurses to treat, and diagnose patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating, hospitals, physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
