Job Description
This is a travel-based, per-diem position – all assignments will require some type of travel to a Home Health agency being surveyed for accreditation/certification.
CHAP Site Visitors are responsible for conducting as well as the planning, organization, and coordination of community-based healthcare accreditation and/or certification surveys. These visits must be performed in accordance with the CHAP Standards of Excellence. The visits must be executed in compliance with CHAP policies and procedures that govern the accreditation/certification process and must ensure confidentiality except where required by law.
Successful candidates must have an expert level knowledge of the CMS Home Health Conditions of Participation and resources related to the CoPs. A knowledge of the CHAP Standards of Excellence is a plus. This position requires a high level of detail orientation and ability to deliver accurate and complete survey documentation. Must be able to work autonomously.

The position requires a bachelor’s degree, Registered Nursing license, a minimum of 5 years’ experience in a compliance heavy role in Home Health industry. At least 1 year of experience in a leadership role (responsible for leading a team/direct reports).
Must be available a minimum of 10 survey days per month (10 days does not include the travel days) and 13 days to qualify for benefits eligibility. You tell us the days you want to work!
Pay structure is as follows:
Survey days: $402/day
Travel days: $165/day (1 day per assigned survey)
Site Visit prep time: $75/per site visit
Other: $60/per day in meal per diem per site visit day, $25 per day in meal per diem for travel day.
Minimum weekly pay: $1651.00 including meal stipends *(based on a three-day workweek and one travel/prep)
Travel: All airfare expenses are company paid; other travel expenses are reimbursed including mileage at government rate. Employee earns all travel credits (airline, hotel, rental car points).
Benefits: Employees working a minimum of 13 survey days per month are eligible for medical, dental, vision, and 401k benefits. This position does not accrue paid time off.
