Home Health RN per diem

**Job Description:**

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team on a per diem basis. As an RN, you will be responsible for providing comprehensive nursing care to patients within the comfort of their homes. Your role will involve assessing patient health, developing care plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

**Key Responsibilities:**

- Conduct initial and ongoing assessments of patients' health conditions
- Develop and implement individualized care plans based on assessments and physician recommendations
- Administer medications, treatments, and therapies as prescribed by the physician
- Educate patients and their families on disease management and self-care techniques
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care effectively
- Maintain accurate and updated patient records and documentation in accordance with regulatory standards
- Adhere to ethical and legal nursing practices and standards

**Qualifications:**

- Current and valid RN license in the state of California.
- Minimum of 2 years of experience in nursing, preferably in Home Health or a related field
- Strong clinical assessment and critical thinking skills
- Proficient in providing skilled nursing care and administering medications and treatments
- Excellent communication and interpersonal skills, with the ability to empathize with patients and their families
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Knowledge of relevant healthcare regulations and compliance standards

**Schedule:** Per diem, flexible scheduling based on availability and patient needs

**Location:** Santa Clarita and San Fernando Valley
