We have immediate openings for RN Case Managers. If you have a desire to use your skills and experience to help others, this is the place for you!
Responsibilities:
Review, QA, correct/revise, and approve Oasis documents, Skilled Nursing (SN) Assessments/Discharges, and SN visit notes, and complete other required documentation that falls within your assigned case management alphabet.
Supervise and communicate with field staff
Advocate for patients and their families to provide the highest possible level of care and satisfaction.
Work with supervisors, patient care coordinators, and administrative staff to resolve patient care issues as needed.
Qualifications:
Valid Texas RN License
Successful completion of Background Screening
Previous Home Health experience (preferred)
Valid Driver's License and Proof of Auto Insurance
