- As an Area Clinical Manager (RN), you will support the DON/ADON/Regional Clinical Manager/DOCS and other relevant clinical management team. You will coordinate and communicate information relevant to the patient care process. This role may schedule patient care staff, receive referrals and physician orders and perform patient visits per productivity standards.
- Reviews available patient information related to case, including home visits, to determine home health care needs. Assigns appropriate home care personnel to case as needed.
- Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals.
- Processing incoming referrals.
- Management and mentoring of Clinical Associates and field staff. Documentation review, including OASIS.
- Managing staff productivity, and overall case management.
- Manage Field Clinicians productivity and report to intake/scheduling dept.
- Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is always available during operating hours to assist clinicians as appropriate.
- Reviews patient's medical diagnosis, procedures, medications, and clinical course.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Attends, runs, and documents case conference meetings with organization personnel to facilitate coordination of care.
- Participates in quarterly record reviews and communicates findings and recommendations to ADON/DON/Regional Clinical Manager/DOCS and appropriate organization personnel.
- Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.
- Assists ADON/DON/Regional Clinical Manager/DOCS in the planning, implementation and evaluation of in-service and continuing education programs.
- Complies with accepted professional standards and principles.
- Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system. Performs other duties and activities as delegated by the ADON/DON/Regional Clinical Manager/DOCS.
- Communicates pertinent patient information to the patient's physician, payor source and/or appropriate clinical staff.
- Receives and distributes information regarding lab test results and other pertinent reports to the patient's physician and appropriate clinical staff.
- Contacts physician for any orders needed as it relates to changes in patient's condition identified in High Priority Clinical Events queue and documents the physician orders received.
- Maintains various reports and records for the Director and Clinical Management team.
- Must be a registered nurse (RN) licensed in Texas (Required)
- Proficiency in HCHB R2 workflow
- Minimum of two (2) to three (3) years of clinical and management experience as a RN
