- On-calls - should be available for emergencies and questions by staff 24/7.
- 60% - 70% in the office, remaining percentage will be supervisory visits on the field.
Responsibilities:
- Adhere to, uphold, and champion our Core Values.
- Ability to supervise, guide, and develop nursing skills and performances.
- Coordinates with management on the development of KPIs to be updated on a regular basis.
- Coordinates with Quality Assurance and Management to develop a QAPI program.
- Coordinates with clinical teams to achieve goals as set forth by the QAPI program and KPIs of the agency.
- Identifies and educates clinical staff on updates, changes, or edits to state, federal, and company policy and procedures.
- Ensures and educates awareness and understanding of HHCAHPS and own role in driving HHCAHPS scores.
- Identify and implement training with clinical staff to improve patient satisfaction and HHCAHPS.
- Coordinates with corporate and local management on proper utilization and frequency standards as dictated by coding regulations.
- Responsible for the orientation of new patient care personnel.
- Administer case management & follow-up skilled nursing visits in the patient’s home. Performs the services of a Registered Nurse as necessary.
- Identify and educate on OASIS-E challenges, particularly around VBP/Star OASIS items.
- Investigates and report any problem relating to patient care and/or employee well-being.
- Attends and assists with pertinent continuing education programs other than routine in-services and shares information with clinicians.
- Ensures that all necessary supplies and equipment are available.
- Ensures that documentation is completed in accordance with Agency policy.
- Assists with staffing visit assignments and on-call.
- Assists and performs supervisory visits to help understand patient satisfaction and training needed for clinical staff.
- Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
- Assists in the coordination of interdisciplinary team services.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
- Can act as a backup administrator if necessary.
- Other duties as assigned by the Clinical Manager and Administrator.
- Must have knowledge of Oasis E.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals to enlist the cooperation of many people to perform/achieve a common goal.
- Must be solution-oriented, resilient, and persistent on all levels relating to business, social situations, and clinical challenges.
- Must possess the ability to work tactfully with personnel, guests, family members, visitors, government agencies/personnel, and the public.
- Must demonstrate excellent interpersonal skills and ability to handle difficult situations with tact and finesse.
- Hold a current Registered Nurse (RN) in the state of Arizona.
- One year of Home Healthcare experience is required.
- Possess and maintain a current CPR certification
- Basic computer skills.
- Experience in wound care, vacs care, ostomy care, and IV administration is preferred.
- Demonstrates ability to work independently with minimal supervision.
- Must have a valid driver’s license, and auto insurance.
- Must have a background check
- Required to present a negative TB test and other tests as required by agency policy
- Must be able to do fieldwork
- Must have excellent written and verbal skills
- Must be 24/7 available to answer questions from staff
- This position is 60%-70% based in the office, and will also do fieldwork.
