Home Health Physical Therapist (Pt)

Position:

• Physical Therapist (PT) for Home Health in South LA and surrounding areas
• $85-120 per visit

** Flexible Schedule** As an independent contractor, you can choose your patient volume and schedule patients on days and times that work with your schedule.

**BONUS** You can get a $500 bonus for referring a physical, occupational and speech therapist!

Areas of Service:

• South LA, South Central LA, Downtown LA, Leimert Park, Ladera Heights, Inglewood, Westmont, Baldwin Hills, Crenshaw, USC Area

Physical Therapist Responsibilities:

• Provides physical therapy services to patients according to a written individualized plan of care
• Initiates physical therapy program and instructs other personnel and/or family/caregivers in certain phases of physical therapy with which they may work with a patient
• Prepares and submits clinical and progress summaries based on the attainment of goals
• Provides physical therapy consultation to families/caregivers, as appropriate
• Provides in-service education programs for nursing personnel as needed
• Provides supervision of physical therapy assistants and hospice aides, as appropriate

Physical Therapist Requirements:

• 1 year of Home Health experience
• A valid practicing license from the State of California
• Must have business license
• Professional liability insurance
• CPR Certification
• Driver’s license
• Auto insurance
• Physical Exam
• TB Clearance

Job Type: Contract

Pay: $90.00 - $120.00 per hour
