HOME HEALTH LVN CASE MANAGER - OFFICE

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a HOME HEALTH LVN CASE MANAGER (OFFICE). In this role, you will work collaboratively and play a crucial role in coordinating and managing the care of patients receiving HOME HEALTH services.

​Responsibilities:

• Coordinate and manage the delivery of healthcare services to patients in their homes.
• Collaborate with the healthcare team to develop and implement comprehensive care plans based on patients' needs.
• Monitor patients' progress and update care plans accordingly.
• Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes.
• Communicate effectively with patients, families, and other healthcare professionals.
• Report changes in patients' conditions to the healthcare team and adjust care plans as needed.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive care.
• Adhere to all regulatory requirements and ensure that care provided meets established standards.
• Effectively manage time and prioritize tasks to ensure efficient and timely delivery of care.
• Advocate for patients' rights and ensure that their needs are met.

​Qualifications:

• Graduate of an accredited school of Nursing
• Possess a valid, current, unrestricted LVN license able to practice in CALIFORNIA - Home health experience preferred but new grads welcome!
• Strong knowledge of medical terminology, tools, equipment and nursing principles
• Ability to work independently, collaboratively and make sound decisions
• Compassionate and caring demeanor
• Excellent written and verbal communication skills
• Strong organizational and time management skills
