Job Description:

As a Full-Time Home Health LVN  you will play a crucial role in providing exceptional healthcare services to  patients in their homes. You'll have the opportunity to develop strong patient relationships and deliver care that promotes independence and overall well-being.

🔍 Responsibilities:

• Conduct in-home patient assessments and develop individualized care plans.
• Administer medications, perform wound care, and assist with various medical procedures.
• Monitor patient vital signs and document changes in health status.
• Educate patients and their families on proper self-care techniques.
• Collaborate with the healthcare team to ensure a coordinated and holistic approach to patient care.
• Maintain accurate and up-to-date patient records.
• Provide emotional support and companionship to patients.
• Adhere to all applicable laws, regulations, and company policies.

📋 Requirements:

• Current LVN licensure in the state of Texas.
• Minimum of 1-2 years of experience as an LVN, preferably in Home Health.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Reliable transportation and a valid driver's license.
• Ability to work independently and as part of a team.
